I agree to my child taking part in 1-1 monologue sessions with Daniella Sanderson. I understand I will be advised when and where the session will take place and be a part of the communication before the session. I understand I must accompany my child (If under 18) to the session to comply with safeguarding regulations.

I agree to my child receiving medical care in the event of an emergency. I give permission to a member of the Flying Dove Theatre team to sign any consent forms required by medical professionals if the waiting period to acquire my signature is unadvised by hospital staff.

I accept that Flying Dove Theatre holds public liability insurance which covers my child whilst in class. However, I discharge the Flying Dove Theatre team from any liability caused by any careless behaviour carried out by my child.
