Summer Fun Day Participation Form

Dear Parent/Guardian,

We are so excited to be performing at the Summer Fun Day at St Peter’s Church on Saturday 20th July 2024. Everyone has been working so hard getting ready for the performance! The event runs from 12.00-3.00pm with many activities and stalls for you to take part in. Please arrive to the venue by 12.30pm to make sure we are prepared and ready to perform at approximately 1.10pm. As this is a public event, we cannot supervise all children at all times so we kindly ask that you stay with your child on the day and supervise them for the duration of the fun day. Please fill out the form below to officially sign up for the performance. Any questions do let us know.

Flying Dove Theatre cannot supervise your child at this event. I understand I must stay with my child for the duration of the Summer Fun Day and supervise them at all times.

I accept that Flying Dove Theatre holds public liability insurance which covers my child during the performance. However, I discharge the Flying Dove Theatre team from any liability caused by any careless behaviour carried out by my child.
